    • When you have completed filling out the form, present it to the Borough Clerk, along with a valid registration certificate issued by the Legalized Games of Chance Control Commission. Caution: The Governing body of the must approve your license at an open public meeting prior to the license being issued. You cannot begin any portion of the bingo or raffle prior to physically receiving your license from the municipal clerk.
  • SAMPLE RAFFLE TICKET AWARDING MERCHANDISE AS A PRIZE 
    • Organizations registered with the Legalized Games of Chance Control Commission use this form as a guide to ensure that all of the required information is printed on the ticket sold to participate in an Off Premises Draw Raffle awarding merchandise as a prize. Two copies of the sample ticket should be submitted to the municipality along with the Raffle Application.
  • OFF PREMISE 50/50 SAMPLE RAFFLE TICKET
    • Organizations registered with the Legalized Games of Chance Control Commission use this form as a guide to ensure that all of the required information is printed on the ticket sold to participate in an Off Premises Draw Raffle awarding cash as a prize. Two copies of the sample ticket should be submitted to the municipality along with the Raffle Application.
