Proof of identification is required.

BLOODWORK REQUIREMENTS 

Pre-registration with the Westfield Health Department beginning

 November 1, 2010.

Registration Location:

425 East Broad Street in Westfield, NJ.

ONLY residents of Fanwood, Garwood, Mountainside,
New Providence, Roselle Park, Springfield, Summit and Westfield are eligible for this service.
